Acceptance Hours:
Monday through Friday - 9:00 a.m. to 3:30 p.m.
U.S. Passport Book: Valid for travel by air, sea or land.
U.S. Passport Card: Not for travel by air. Valid for land and sea crossings.
Passport (Under 16)
Items to bring
- Birth certificate with a raised seal
- Both parents MUST be present to sign. If the second parent is not available to sign, the appearing parent must present evidence that he/she has sole legal custody of the child or written consent of the other parent with signature notarized.
- 2"x2" Duplicate Photos (can be obtained for $10 per person at the Clerk's Office)

- FOR PASSPORT APPLICATION Fees (payable to the U.S. Dept of State) Personal Checks & Money Orders Accepted ONLY -No Credit Cards Accepted
- Acceptance agent fees, postage, and photos can be paid with cash, check, money order, or credit card
- Passport Book Fee $100.00 (Payable to U.S. Dept. of State) + Acceptance Agent Fee $35.00 + Postage (fee varies) (Payable to City of Flat Rock) - Effective February 20, 2022
- Passport Card Fee $15.00 (Payable to U.S. Dept. of State) + Acceptance Agent Fee $35.00 + Postage (fee varies) (Payable to City of Flat Rock) - Effective February 20, 2022
